After you have installed Acrobat Reader - come back to this page and click on the link below to view and complete the application form. |
| 2. Every field must be filled out completely before submitting this application to the Oklahoma Tax Commission. The following list details every item that must be completed: |

| check either the "Original" box for new applications or the "Renewal" box for those of us who already have a BPOE plate |
| Renewal Applicants need to indicate in the box directly below the "Renewal" check box what their current plate number is or says. |
| The box for the Elks Associatiion (ELN) should already be checked - do not change this |
| (The following information can be found on your vehicle's registration paperwork) |
| Oklahoma Title Number (Not your VIN Number) |
| Year and Make of the vehicle for this license plate |
| Regular (current) license Plate Number |
| Decal # on your current license plate |
| Decal expiration date on your current license plate |
3. Once you have completed the application form - sign it. |
| 4. Make a photocopy of your current Elks membership card. |
| 5. Make your check or money order payable to the Oklahoma Tax Commission. Your Driver's License number must be on it. |
| 6. Send both the completed application and the photocopy of your membership card along with your payment ($17 for new or $16 for renewals) to: |
